At a guess I’d say her tattoo artist, her doctor and either Carly or Ace.
What about Carly’s father?
Also note how she said “And only three men alive have ever seen it all.”
Tattoos of that sort are not generally done until you are fully an adult, she was still too young when she gave birth to Carly, and it does not sound like she saw Carly’s father after becoming an adult.
I would have thought both Carly and Ace. Recently, at their pool and or hot tub.
We might not have seen both Carly and bikini-less Aoi in the pool at the same time on-page, but it seems likely to have happened.
Just from Luna’s aging we can see more time is passing here than we are strictly shown.
Ace peeks, but never stares or leers, which is part of what women like about him. It is obvious that he finds women attractive but he respects the fact that their nudity is not an excuse for him to stare, but it would be challenging to see that entire tattoo in a quick glance. The tattoo artist would have had to see what he was doing, so he obviously has seen it, and it is difficult to do a complete examination medically, so her family doctor has likely seen it. I would judge it more likely that Aoi would not be naked in front of her father as it sounds like he has been with women who’ve given birth and Aio’s stretch marks would be hard to hide. This leave Carly as the most likely 3rd male
